Application Instructions: *  Do not click on the signature line. *  When application is complete, click E-SUBMIT.  A message box will appear.  Click on your appropriate e-mail.  An e-mail message box will appear. Click on "send." *   The application form will appear on your screen.  Click PRINT if you choose to have a hard copy.  Be sure when you are finished to RE-SET to remove your information from the form. *   You can print the completed form and e-mail or fax to 574-631-0856.  *   Be sure to include your essay and unofficial transcript as well.
