How do I waive out of the school sponsored insurance plan?

You must submit proof of a comparable insurance plan with an American-based insurance company each school year.

What is considered a comparable plan?

  • Yearly maximum of 10,000 per injury or illness.
  • Deductable is $200.or less
  • Co-insurance reimbursement rate of 90% or greater.
  • Pharmacy benefits included
  • inpatient/Outpatient benefits with no daily limitations
  • It must be an American-based plan

How much will my prescription cost with the new United Healthcare (UHC) Student Resources policy for 2007 - 2008?

At the Health Center Pharmacy, co-pays are $25.00 for name brand and $15.00 for generic. If your medication has a generic and you choose the name brand, the co-pays will be $50.00.

What drugs are NOT covered by UHC Student Resources Insurance at "Out of Network" Pharmacies (any Pharmacy except the Notre Dame Health Services Pharmacy)/

Drugs not covered include the following:

Do you accept insurance?

The only insurance that we take is the Notre Dame School Sponsored Plan with UHC Student Resources Insurance. If you have private insurance, we will provide you with a detailed receipt to submit for reimbursement to your insurance carrier. We can also provide a detailed statement upon request.

What is the deductible amount?

It is $100.00 per injury/illness. Maximum deductible for policy year is $200.00

When will I be issued an insurance card?

A temporary insurance card is in the back of the insurance brochure. Remove it, place your signature on the line, and carry it with you. In about a month after enrolling, you should receive a permanent card from UHC Student Resources.

What hospital do I use?

Memorial Hospital is the preferred provider.

How much is the annual premium for the student insurance?

For the 2007-2008 school year, the annual premium is $1,389.00. There are optional

If I have private insurance do I have to purchase the Student Insurance?

No. Undergraduate students are not required to carry insurance and therefore do not need to show proof of insurance. However, all international students and degree seeking graduate students are required to carry health insurance. If you have a private plan that is comparable to our plan from an American-based company, you need to sign a waiver form and provide a copy of your current insurance card to us before the waive out deadline.. This needs to be done each Fall semester between 8/15 and 9/15. If arriving in January for the Spring/Summer semester, you have between 1/15 and 2/15. If coming to campus for the Summer session, you have from 5/15 - 6/15 to waive out. There are no exceptions.

If I have private insurance do I need my Insurance card to be seen at the Health Center?

No. When you are seen at the Health Center, we will put any charges incurred on your student account and send an itemized bill to you to file with your insurance carrier. If you have a pharmacy card , you can ask the physician to give you an outside prescription to take to a pharmacy off campus.

Can I be seen at the Health Center if I don't have the student insurance?

Yes. As long as you are a registered student at the University. If your status changes, such as graduation or a leave of absence, then you can not be seen at the clinic.

Does the student insurance cover Immunizations for students?

No. It is under the exclusion of preventative medicines or vaccines.

You can visit our web site or pick up a brochure about the student insurance coverage at the Health Center. If you cannot pick one up, you may call and request it at (574) 631-6114.
