Undergraduate Studies in the College of Science

The College is making available some standard student forms which can be submitted online. Those forms are listed to the left. The forms Overload Request, Part-Time Status Request, Pass/Fail Request are available for submission on-line until 11:59 p.m. on the 5th class day.  If you wish to make any of the requests on the 6th class day, you must do so directly to the Associate Dean’s Office, 174 Hurley Hall.

Any form not listed must still be filled out in the paper version and sent to our office at:
