Use
Updated Proposal Routing Form dated May 2006
When submitting a proposal, please use the latest version of the Proposal
Routing Form. The latest version is dated May 2006 and includes additional
items under the signature and approvals that the National Institutes
of Health are mandating as a compliance requirement. This updated form
should be used for all proposals coming to the Office of Research.

R03,R21,R33,& R34 for JUNE 1, 2006 SUBMITTED VIA GRANTS.GOV
ALL
APPLICATIONS SUBMITTED IN RESPONSE TO R03, R21, R21/R33 and R34 funding
opportunity announcements (FOAs) with submission deadlines of June
1, 2006 or later must be submitted electronically. However,
you may have missed the May 4th notice in the NIH Guide indicating
that there were a number of active R03, R21, R21/R33, R33 and R34
FOAs that had not yet been updated to reflect the new electronic submission
process. The notice immediately expired any R03, R21, R21/R33 and
R34 announcement (or portions of announcement) that still referenced
paper submission for these mechanisms. Applicants were encouraged
to use parent FOAs (referencing expired FOA in the cover letter) or
to contact the NIH Institute or Center (IC) that issued the specific
expired FOA for guidance.
If you are planning on submitting one of these NIH mechanisms for
the June 1, 2006 deadline, please notify the Office of Research immediately
at researc2@nd.edu so that
we can be prepared and we can ensure that you have the tools you need
to make this process of submitting through Grants.gov a good experience.

Implements New Compliance Requirement Beginning May 10, 2006
Effective
with PHS398 Applications prepared for submission/receipt dates on/after
May 10, 2006, the signature of the Principal Investigator
is no longer required as part of a submitted application. Instead,
a new compliance requirement is now implemented whereby the applicant
organization agrees to secure and retain at the organization a written
assurance from the Principal Investigator (PI) prior to submitting
an application to the PHS. Organizations
must retain a unique signature and date for each submitted application. This
assurance must be available to the sponsoring agency or other authorized
HHS or Federal officials upon request. Such an assurance must include
at least the following certifications:
• that the information submitted within the application is true, complete
and accurate to the best of the PI's knowledge;that any false, fictitious,
or fraudulent statements or claims may subject the PI to criminal, civil, or
administrative penalties; and
• that the PI agrees
to accept responsibility for the scientific conduct of the project and to
provide the required progress reports if a grant is awarded as a result of
the application.
NOTE: When multiple PIs are proposed in an application, this assurance
must be retained for all named PIs.
In order for the University of Notre Dame to accomplish this, the Office
of Research will be modifying the Proposal Routing form to add the three
statements above so that when the PI signs off on the routing form they
will be assuring their compliance with these statements.
An updated Proposal Routing Form will be posted on the Office of Research
website in the forms section prior to May 10, 2006.

Requiring Updated Versions of Forms
The National Institutes of Health will be requiring updated forms according
to the following schedule:
• PHS 398 forms for
submissions after May 10, 2006 require the 4/2006 versions of forms
• PHS 2590 forms will also require the 4/2006 versions of forms for submission
dates beginning June 1, 2006
