Request for Testing Accommodations Form

Please be sure that you have read and understand the Testing Accommodations Policy prior to submitting the following form.

Each semester, students must request that the Coordinator prepare a Request for Testing Accommodations form for the instructor of each class in which they are requesting accommodations.

After filling out this online form, students will need to pick up a completed Request for Testing Accommodations in the office and discuss accommodations with their professor(s).
