The most frequently asked questions concerning animal protocols, the animal facility, and personnel preparation for animal work will be addressed.This page is designed to provide information on multiple topics and links to pages with more detailed information. Personnel Amendment forms are online Here  under Compliance forms - the fourth one listed  note: All personnel working on an animal protocol must be listed on the protocol. The amendment form has a section for adding personnel or deleting them from the protocol.

Investigators using controlled Substances need to contact Risk Management and Safety and apply for both State and Federal Controlled Substance Licenses. The button below will take you to a powerpoint slide show that reviews the resposibilities of the DEA license holder.

Training

Participation in the Occupational Health Program (OHP) is mandatory for all persons listed on animal protocols. Click the button below to view the powerpoint slide show and to access the OHP handout and OHP Medical/Occupational health Forms. OHP effective May 11, 2012 will be administered through the Office of RM&S. Contact Jenna Leevy at 631-5037 with any questions concerning Occupational Health.
