As of May 1, 2008, controlled substances are not available from FLSC.  All researchers using controlled substances MUST have secured a Federal DEA Controlled Substance license and an Indiana State Controlled Substance license.  Controlled substances used in each lab will be the responsibility of the DEA license holder in that lab.  The slide show linked below details the responsibilities and duties of the DEA license holder.  For additional information, contact Mike McCauslin in the office of Risk Management and Safety - phone 631-5037.  For your convenience, forms for inventory and use logs are available in both .pdf and .doc formats on the Forms page.
