Guidelines for nomination are:

• all library faculty and staff are eligible except for individuals currently under corrective action
• any employee may nominate any other employee, but no employee should nominate more than two individuals
• no individual may win an award in two consecutive years
• awards are for individual employees, for teams, use the group recognition form

Complete this Performance Recognition Award Nomination Form or send a brief letter of nomination to Diane Kennedy (116 Hesburgh Library or kennedy.75@nd.edu) by December 5, 2003.

All nominees will be considered in one pool for this year's awards, each award called a Performance Recognition Award.
