Special Instructions for Summer Toledo Program Applicants ONLY

INSTRUCTIONS FOR TOLEDO SUMMER 2006 PROGRAM APPLICANTS

Applicants to the Toledo Summer program must submit an application to Notre Dame's International Study programs Office:

1. ISP On-line application for Notre Dame:
   • COMPLETE ONLY THE PERSONAL INFORMATION SECTION AND WRITE A SHORT ESSAY.
   • You do not need to ask professors or rectors for references. Enter Anne Hayes for the first academic reference and Sarah Baer for the second academic reference.  Enter Anne Hayes as the alternate rector reference (do not put a name for the rector reference field).
   • You do not need to get academic or Dean approvals for summer programs.
   • PLEASE DO PRINT AND COMPLETE THE PARENTAL CONSENT FORM. The electronic application and parental consent forms are due March 1st, 2007.
• You must bring the parental consent form to 152 Hurley by March 1st, 2007.
