Special Instructions for Summer Puebla Program Applicants ONLY

INSTRUCTIONS FOR PUEBLA SUMMER 2006 PROGRAM APPLICANTS

Applicants to the Puebla Summer program must submit two different applications to Notre Dame's International Study programs Office:

1. ISP On-line application for Notre Dame:
   • COMPLETE ONLY THE PERSONAL INFORMATION SECTION AND WRITE A SHORT ESSAY.
   • You do not need to ask professors or rectors for references. For the first academic reference, enter the name Anne Hayes. For the second academic reference AND as an alternate rector reference, enter the name Sarah Baer.
   • You do not need to get academic or Dean approvals for summer programs.
   • PLEASE DO PRINT AND COMPLETE THE PARENTAL CONSENT FORM. The electronic application and parental consent forms are due March 1, 2006 .
   
   
2. Universidad de las Americas (UDLA) Centro Internacional de Lengua y Cultura (CILC) application:
   • Print page 1 and 2 of the CILC application. Complete these pages and bring them to 152 Hurley by March 1, 2006.
   • You must also attach an official transcript, a copy of your health insurance card, and a copy of the first page of your passport.  We will notarize that in our office and send applications to Mexico for you.
