If a student admits to having violated the Code, and if that student and the instructor can agree on an appropriate punishment, the instructor can complete an Honor Code Violation Report indicating the nature of the violation and the agreed-upon penalty.  Guidelines For Selecting Honor Code Sanctions serves as a guide to help the instructor and student agree upon an appropriate penalty. The Honor Code Violation Report, signed by both the instructor and student, must be mailed to the Associate Provost who co-chairs the University Code of Honor Committee.
