AUSTRALIAN VISA INFORMATION
You will need an Australian visa in order to study at Notre
Dame Australia. Please pick up a Visa Application Packet from the
ISP office at 109 Hurley and start working on the materials ASAP.
When you have completed the forms, send the packet to the Australian
Consulate for your state of residence.

You should include the address of the University of Notre
Dame Australia - 19 Mouat Street, Fremantle, Western
Australia 6160.

Check the dates of the Fremantle program with the ISP office
and include them in your cover letter to the Visa Office.

According to the Immigration Office of the Australian Embassy,
as of 1 April, 1996, you no longer need to send in the chest X-ray, unless
there is an abnormality in it. You do still have to have it done
and send in the radiological report with the rest of your visa papers.

You must include a visa processing fee of US$230.

STUDENT VISA APPLICATION CHECK LIST

Visa Form 157Y: If you
are not a citizen of the US, please contact an Embassy/Consulate General
to find out if you may require a different version of this form;

Visa Form 160 (Radiological
Report): This must be fully completed by the student and
the doctor. The doctor signing MUST BE A QUALIFIED M.D. The
doctor must sign across the top of the photo attached. The doctor
must also place the form in the attached envelope and seal it, signing
over the seal as well. If you have a pre-existing condition, or any
other abnormalities are revealed in the Radiological Report, you will need
to request a Form 26 from the Visa Office. You will need to have
a physical exam, and the Form 26 needs to be completed by an M.D.
A separate, detailed written report will be requested for any health abnormalities,
and a doctor's physical may be requested.

Passport: A passport,
signed and valid through the length of your stay in Australia is required.

Fee of US$200-US$230*:
Bank certified checks or money orders should be made payable to the Australian
Government. Or if the student wants to pay by credit card, fill out
the form enclosed in your packet. Only students who are participating
in a high school Rotary exchange are exempt from paying the processing
fee. *Check with the International Studies Office
for the exact fee amount as this is dependent on the prevailing exchange
rates.

Confirmation of Enrollment:
A letter issued by the university or school in Australia to which the student
has been accepted and paid enrollment fees. The student should make
a copy of the letter (AND ALL OTHER MATERIALS SUBMITTED) and forward the
original to the correct visa issuing office for your state of residence.

Self-Addressed, Stamped (Not Metered)
Envelope: Include for the return of your passport and visa.
It is EXTREMELY IMPORTANT THAT YOU INCLUDE A STURDY, SELF-ADDRESSED, STAMPED
ENVELOPE (FOR THE CORRECT AMOUNT OF POSTAGE) TO PREVENT THEFT OF YOUR PASSPORT.
IT IS HIGHLY ADVISABLE THAT YOU INCLUDE A PRE-PAID, TRACEABLE, OVERNIGHT
ENVELOPE FOR THE SECURE AND TIMELY RETURN OF YOUR PASSPORT AND VISA. Please
make sure the overnight envelope is filled out correctly, including zip
codes and telephone numbers, or it will be useless.

Cover Letter: Make
it brief, but state your intended departure date to Australia.
Sending Forms in to the Immigration
Office

Allow 21 working days for processing. In time-sensitive
cases, send in what has been completed to date then send in remaining forms
when completed.

USE A FORM OF TRACKABLE MAIL (Federal Express, UPS, Airborne
Express, USPS Overnight Mail, etc.). USPS CERTIFIED MAIL IS NOT
TRACKABLE. On the outside of the package, print largely and legibly
the students name and departure date.
NOTES:

The issuance of a student visa is dependent upon a satisfactory
health clearance by the Australian Government.

The student should submit all completed forms to her/his
appropriate issuing office according to state of residence.

If you require an extension of your visa while in Australia,
please lodge an extension application with the nearest Department of Immigration
and Multicultural Affairs (DIMA) office IN AUSTRALIA.
