INSTRUCTIONS TO RADIOLOGISTS

Please use the following procedures when completing medical forms for applicants:
 

• SIGN across the applicant's photograph.
• SIGN and DATE the application form with your full medical title.
• Provide a business card with the correct telephone number and address.
• Place a mark in each appropriate box for questions 1 to 7.
• Please provide a separate report if any abnormality is shown which details diagnosis, management and prognosis of the condition.
• Place all documents within the envelope and SIGN and DATE the sealed flap.

NOTES:

• All applicants 16 years of age and over require a radiological report.  However, only send the X-ray film if an abnormality is detected.
• These forms may only be completed by a MEDICAL DOCTOR (MD).  Any MD in North America can perform these examinations.
