Notice and Waiver

Due to the age of the premises at 857 Forest Ave, South Bend IN 46616 the possibility of leaded paint being present cannot be ruled out. The lessor assumes no responsibility for possible damages caused by leaded paint, including bodily injury or death of the lessee. By signing below, lessee acknowledges receiving a copy of the EPA's pamphlet on lead poisoning and agrees to hold harmless the lessor for any injuries which may arise as a consequence of leaded paint on the premises.
