Patient was admitted 12 March 1999, 10:00 am, for chelation therapy as ordered by Dr. C. Hadley. Initial blood workup (10:30 am blood draw) shown below:

Calcium Disodium Versenate (200 mg/ml) was mixed 1:1 with 1% Procaine. 1.6 ml was given intramuscularly at 12:00 noon, 8 pm, and 4 am. Copious fluids given by mouth. Patient complained of muscle pain after injections, children's Tylenol was given every 6 hr.

13 March, 1999. In morning, patient's parents reported some flecks of blood in patient's urine. Vital signs normal. Blood work done on 6 am blood draw:

13 March con'd. IM calcium disodium versenate administered at 12 noon, 8 pm, and 4 am, along with children's Tylenol and fluids by mouth. Patient complains of pain "inside" on urination, some blood in urine throughout day.

14 March, 1999. Blood work done on 6 am blood draw:

14 March 1999 con'd. Dr. Hadley orders early end of chelation therapy after 12:00 IM injection. Patient kept in ward to monitor renal function.

16 March 1999. Renal function recovered, patient discharged at 10 am. Blood work done on 6 am blood draw:
