Please download, print, and fill out both the Best Buddies Membership Application and the Matching Survey.

Those who wish to be associate buddies need only fill out the first form, however those who want to be paired MUST fill out both.
Both documents should be turned into the Best Buddies mailbox which is located in the Club Resource Center on the 3rd floor of LaFortune Student Center.

** Both new club members and returning club members must fill out these forms. **
