TO:               University Administrators and Administrative Staff

FROM:           John F. Affleck-Graves, Executive Vice President

DATE:           March 4, 2009

SUBJECT:      2008/2009 Potential Conflict of Interest Disclosure

In accordance with best governance practices, the University biennially contacts its administrators and administrative staff in order to document activities and relationships which might constitute a conflict of interest under the University’s Conflict of Interest Policy.  The purpose of this process is to ensure that we capture and maintain current information on such relationships, as well as determine whether any such relationships constitute unacceptable conflicts of interest.  Although our independent certified public accountant, PricewaterhouseCoopers LLP, has never indicated that the University’s administrators or administrative staff are involved in unacceptable business practices, this circulation is consistent with the business ethics and practices we value at Notre Dame.

For your convenience, the Policy and Potential Conflict of Interest Disclosure Statement can be accessed by clicking the relevant links in this webpage.  Please review the Policy, then print the Statement (on both sides of the paper, if possible), complete, and return the Statement to Audit & Advisory Services (AAS) by Friday, March 20, 2009, indicating any possible or potential conflicts.  To help ensure the accuracy of our records, please also indicate the existence of any known conflicts of interest which have been previously approved, as explained in the Policy.  The signed Statements for all administrators and administrative staff will be maintained in AAS.  We will continue to issue formal, periodic solicitations as needed; however, any potential conflicts of interest should be brought to the attention of AAS, the Office of the General Counsel, or my office as they arise.

Completed Statements should be returned to Audit & Advisory Services, 402 Grace Hall, Notre Dame, IN  46556.  If you have questions pertaining to the Policy, Statement, or the compatibility of your activities with the requirements of the Policy, please contact Jeremy Miller at 631-6616 (miller.309@nd.edu) or Marianne Corr at 631-6411 (mcorr1@nd.edu).  Thank you in advance for your cooperation in this matter.
