Survey Information
The annual survey forms are sent to the Deans, Directors,
and Building Representatives. All requests must be approved,
prioritized and signed by the appropriate dean or director.
The form is designed to provide detailed descriptions relating to the work needed. This information should be completed by the contact person for the proposed project. NOTE: Requests for facilities for new faculty must include as much detail as possible; include the faculty name and the phone number of the contact person so that further data may be gathered by the project coordinator for estimating and design purposes. During the estimating process, Project Coordinators from Facilities Operations will meet with the contact person for further clarification. It is important that all information which impacts these projects be documented. This will eliminate delays or dramatic changes in the scope of work originally requested and previously estimated. It is very important to know all requirements during the estimating process. Our allocation is built upon these estimates; any change requests must be reviewed by this office and may require deferment of the project, or cancellation of other requests. All anticipated projects for new faculty positions must be included now, whether or not the position has been filled.
It is very important that all requests be submitted by the deadline in order to be included in the estimating process and subsequent review meetings prior to submission for funding. As in the past, if our budget allocation cannot support all project requests, survey items will be deferred based upon priorities submitted.
Please refer all maintenance repairs throughout the year to Facilities Operations by work order, or telephone, in case of an emergency. Any projects which are to be funded with departmental monies must first be reviewed by this office prior to obtaining estimates. This allows us to maintain the facilities data base appropriately and assures that all work requests do not conflict with other projects which may be planned.
