AME Students:

Periodically we will be asked for student information from campus organizations, clubs and even some companies.  We are always happy to help these organizations providing of course they are using the information for a University approved purpose however the university will not let us release this information unless we have a signed privacy release form on file from our students.  If you would like to be added to the list of students who are allowing us to release the information please complete the form (you can find it through this link: http://ame.nd.edu/undergrad/bb/privacy%20release%20form.pdf)
and return it to me.

Please note:  you only need to complete this form one time while you are  a student here.  If you do not remember if you have completed the form or not you can always email me and I will let you know if I have a form on file for you.
