Postgraduate Service Participation Form

Please submit the following form if you plan to commit to a year or two of service after graduation, even if you are not planning to attend the Send Off Ceremony.

Tickets are required for the Send Off Ceremony, and while the facility is large, seating is limited. In order to ensure that we meet as many of your requests for tickets as possible, we have limited the number of guest tickets to four (in addition to one ticket for the graduate). The tickets will be available for you to pick up at the Center for Social Concerns (in the old security building behind the Rock) on Tuesday, May 13 and Wednesday, May 14 from 9am to 5pm.
