TRAVEL WITH STUDENTS
Faculty/staff planning to travel overseas with student(s), must complete the Faculty Application for Foreign Travel. This form should be submitted to both Julia Douthwaite, Assistant Provost of International Studies and Rev. Mark Poorman, C.S.C., Vice President for Student Affairs 60 days prior to departure. The Offices of International Studies and Student Affairs will determine if the travel is approved. If student participation in the trip is optional, liability waivers using a form approved by the University’s Office of General Counsel must be obtained from every participant before the proposed trip.

TRAVEL ACCIDENT COVERAGE
The University provides $200,000 coverage for faculty and staff in the event of accidental death or up to $200,000 for dismemberment resulting from injuries you sustain while traveling on University business. Your spouse is eligible for $100,000 coverage n the event of accidental death or up to $100,000 for dismemberment resulting from injuries sustained if traveling with you on University business and the spouse travel is approved by the University..
Travel Accident coverage provided by the American International Group excludes countries which are considered “war risk” by the insurance carrier. If the country in which you plan to travel falls in this category, consult the section regarding travel accident coverage for “war risk” countries.

Travel Accident coverage provided by the American International Group excludes countries which are considered “war risk” by the insurance carrier. Should the country you are traveling fall into this category, limited replacement coverage is available through HTH Worldwide.

TRAVEL RECORD
All faculty, staff and student business travel, including international travel, should be documented by using the Department Business Travel Record Form. This report is to be filed with Barbara Davey of the Risk Management and Safety Department at the end of each calendar year.
